Prequalification requirements
- Current general liability, auto and workers' compensation insurance certificates.
- Valid contractor and Licensed Timber Operator credentials where the scope requires them.
- W-9, business license and, for public work, DIR registration.
- Written safety program, EMR documentation and training records for assigned personnel.
- Equipment list with make, model, year and inspection status.
- References for comparable completed scopes.
